Below are links to OTM forms. The forms are available at the Office of Technology Management, but for your convenience, we have also put them here on our web site.

Please read the instructions / or information before completing these forms. It may be helpful to print them out so you can refer to them easily. The form must also be printed (use "Save As," Format: "text," then open using a word processing program), so that a hard copy can be submitted to the Office of Technology Management.
